This guidance provides a framework for recording and measuring progress against personal outcomes for individuals during the care and support process. It applies to individuals who are eligible for a care and support plan and carers eligible for a support plan under the Social Services and Well-being (Wales) Act 2014. These personal outcomes should link to the 24 national wellbeing outcomes. This guidance provides information on defining, recording and measuring personal outcomes; information on measuring progress; and dealing with different interpretations of scoring and measuring.
