Case management

This paper focuses on the evaluation of dementia case management in the Netherlands, as well as factors associated with positive evaluations of informal caregivers. A survey was completed by 554 informal carers. The majority of the informal carers were older (69% was 55+), and female (73%), and often concerned the partner or adult children of the person with dementia.

Older carers of adults with intellectual disabilities experience unique challenges. Outreach initiatives identify a high number who are unknown to support services and a case is made to proactively engage them to assist in future planning for their adult children. An earlier study by the authors suggested that, in Victoria, specialist case management programmes for older carers occupied a unique place within the service system.

This article considers the experiences of a small group of agency care managers (n=23) working in London and the north of England. It was discovered that employment for occupationally migrant care managers can provide access to a potentially rich source of knowledge and skills, as well as a diverse and sometimes stimulating form of irregular employment. However, it was also discovered that work intensification, job insecurity and limited time spent with users and carers were common, and this further intensified the deskilling and lack of discretion which is common for many care managers.

Nurse-led case management programmes have become increasingly popular over the last 15 years. Countries such as the USA, Canada, Sweden and the Netherlands have long running case management programmes in place for frail elderly people. The Department of Health in England has recently introduced a 'community matron' role to provide case management to patients with highly complex long-term conditions; a group that is predominantly comprised of elderly people.

Eighty-three hospital-discharged elders were served by a social work case manager in our 18-month case management project in July 2000. Results indicate that elderly care-recipients utilized less hospital services without sacrificing the quality-of-life of their informal carers. This paper shares the experience in the process of case management interventions and suggests strategies to implement case management interventions successfully.

Objective: To write a narrative review of the role of primary care physicians in the long-term care of people with dementia living at home, with a focus on psychosocial interventions, the provision of information and carer support, behavioural and psychological symptoms and case management.

Methods: The systematic review carried out for the NICE/SCIE Guidelines was updated from January 2006, Cochrane Reviews were identified and other publications found by consultations with experts.
