Social Services and Well-being (Wales) Act 2014: part 3 code of practice (assessing the needs of individuals)

This code provides guidance to the Part 3 of the Social Services and Well-being (Wales) Act 2014, setting out a process for assessing the needs of an individual for care and support, or support in the case of a carer; a process of assessment that will apply to all people – children, adults and carers; and a process of review and re-assessment that will apply to assessments. Underpinning the changes introduced by the Act is the need for more effective arrangements that are used and understood by all practitioners in their work with individuals and to ensure that practitioners work with people to identify what matters to them, and identify their strengths and capabilities is central to the system.
