Making it real for carers

Making it Real for Carers explores what each Making it Real theme means to carers and what this might look like in practice. It has been coproduced with carers and organisations including the Carers Trust and ADASS following a series of workshops across England. 

The template below provides a guide to the approach that could be taken when looking at what's working and what needs improving in relation to Making it Real for Carers. Many organisations will have their own business planning processes and frameworks and this can be used as a checklist to inform those.

Windows of opportunity: prevention and early intervention in dementia: a tool for commissioners and providers

Early intervention is often seen as anticipatory information and support delivered at the earliest stage of dementia, following diagnosis. Prevention encompasses the range of interventions that prevent an increase in need that may tip an individual into a higher or more intensive level of service. This online resource looks a persons journey through dementia and the opportunities for early intervention and prevention along the way.

Re-engineered Discharge (RED) Toolkit

A variety of forces are pushing hospitals to improve their discharge processes to reduce readmissions. Researchers at the Boston University Medical Center (BUMC) developed and tested the Re-Engineered Discharge (RED). Research showed that the RED was effective at reducing readmissions and posthospital emergency department (ED) visits. The Agency for Healthcare Research and Quality contracted with BUMC to develop this toolkit to assist hospitals, particularly those that serve diverse populations, to replicate the RED.

Population assessment toolkit

An interactive toolkit, developed to support the implementation of the Social Services and Wellbeing (Wales) Act, which provides information and advice to support Local Authorities and Health Boards to undertake an assessment of care and support needs and carers' needs in their local area. The toolkit is in three main sections. Section one (the ‘What’ guide) looks at what is required under the Social Services and Wellbeing (Wales) Act, the benefits of carrying out an assessment, analysing the data, and what should be included in the report.

Autistic spectrum disorders toolkit

Autism is a neurodevelopmental disorder that affects at least 1% of the population. There is an urgent need to improve healthcare of people with autism. Research shows that autistic people die on average 16 years earlier than the general population and this increases if they also have learning disabilities. This Autism Toolkit aims to be a 'one stop shop', a user-friendly guide to autism for primary care professionals, people affected by autism, clinical commissioning groups, as well as interested members of the general public.

Self-assessment toolkit: from values to action: the Chief Nursing Officer's review of mental health nursing

This toolkit is intended to complement the report From values to action: The Chief Nursing Officer’s review of mental health nursing. From values to action makes a number of good practice recommendations that apply to nurses working in services for people across the whole age range. Recommendations relate to nursing practice and education and the organisational context in which care is provided. The toolkit is largely for the use of organisations, in consultation with stakeholders – but anyone can use it to reflect on current local nursing-related issues.
