Development and psychometric properties of surveys to assess patient and family caregiver experience with care transitions

Background: The purpose of this study was to develop and administer surveys that assess patient and family caregiver experiences with care transitions and examine the psychometric properties of the surveys. The surveys were designed to ask about 1) the transitional care services that matter most to patients and their caregivers and 2) care outcomes, including the overall quality of transitional care they received, patient self-reported health, and caregiver effort/stress.

Setting up an advisory group to explore the role of partners of adolescents and young adults in cancer care

Minimal research has been undertaken into needs of partners of adolescents and young adults with cancer. However, it is understood to be important for adolescents and young adults with cancer to maintain a connection with healthy peers and that they regard their loved ones, including partners, as valuable to them during their cancer treatment. Research has also suggested that adolescents and young adults consider that loved ones and partners also need support and that this support is lacking in cancer services.

Self‐care experiences of Pakistani patients with COPD and the role of family in self‐care: A phenomenological inquiry

Developing a patient safety guide for primary care: A co‐design approach involving patients, carers and clinicians

Background: Patients and carers should be actively involved in patient safety and empowered to use person‐centred approaches where they are asked to both identify safety concerns and partner in preventing them. Objectives: The aim of this study was to co‐design a patient safety guide for primary care (PSG‐PC) to support patients and carers to address key patient safety questions and identify key points where they can make their care safer.

Functional Limitations Experienced by Older Adults with Complex Care Needs and Its Impact on Access to Community Based Health and Social Care

Introduction: Multimorbidity is often combined with functional limitations among older adults. It is therefore important to ensure the built and social environment encourages older adults to safely age while maintaining independence. Previous findings have focused on the impact of the built environment as a barrier to an active lifestyle, but not specific to accessing community-based health and social care (CHSC). We propose to investigate the impact of patient and caregiver limitations on accessing care, including the impact of the built environment.

Understanding experiences of undergoing transcatheter aortic valve implantation: one-year follow-up

Background: Transcatheter aortic valve implantation (TAVI) is the treatment of choice for frail, older adults with severe symptomatic aortic stenosis. Although research about long-term clinical outcomes is emerging, there is limited evidence from the perspectives of patients and family caregivers on their perceived benefits and challenges after TAVI. Aims: The aim of this study was to describe older adults and family caregivers’ perspectives on undergoing TAVI at one year post-procedure.

The Unmet Needs of Patients and Carers within Community Based Primary Health Care

Introduction: Community Based Primary Health Care (CBPHC) is positioned as the foundation of integrated health systems, intended to support broader goals of population health and health system sustainability. CBPHC moves beyond traditional primary care (a physician visit) to team based care that spans organizational boundaries (such as primary care clinics + community care services).

'After the stroke': patients' and carers' experiences of healthcare after stroke in Scotland

We report findings on patients with stroke and carers experiences of the healthcare system in Scotland after stroke. These findings emerged from data collected in a primary qualitative study exploring patients with stroke and carers perception of a Functional Electrical Stimulation (FES) Clinic.
